File a complaint

Introduction:

Christopher Place Healthcare Review partners with Safe Hotline, a trusted third-party vendor, to provide a secure and anonymous platform for filing complaints or concerns.

Safe Hotline does not conduct investigations into your report. 

Your report will be forwarded to Christopher Place Healthcare Review compliance officer for further investigation. 

Safe Hotline service is a third-party platform that allows you to report anonymously without the fear of retribution.

Ways to File a Complaint:

Safe Hotline provides you with two ways to make a report. 

1. You can type your report at safehotline.com by selecting the ‘Submit Report’ option in the upper right hand corner. 
2. You can also text or call to leave a voicemail at 1-855-662-SAFE (1-855-662-7233). Your voicemail will be transcribed into text so that you can remain anonymous.

Spanish-speaking individuals will only be able to make a report by phone. Instructions in Spanish are provided with this option, but not with the text or website options.

Please do not leave any self-identifying information. However,if you would like to be contacted by the Christopher Place Healthcare Review, include your name and contact information.

Reporting Status:

The status of your report can be checked by selecting the ‘Report Status’ option via any reporting method – text, voicemail or website. 

To check the status and respond to any comments and/or questions made by Christopher Place Healthcare Review, you will need the Company ID number and the Report Key number.
